The referring physician's office or patient should contact Dr. Fitzpatrick's office at (480) 425-8004 to schedule an appointment.
The referring doctor's office should fax (480-425-8002) patient records, a recent MRI radiology report (date of the MRI must be within the past 4 months) and a referral or authorization if required by current insurance company(s).
What happens next?
Dr. Fitzpatrick's office would like the patient to come in 10 minutes early for registration if all forms have not been completed in advance.
MRI films should be hand carried by the patient at the time of the appointment (if x-rays have been completed also, those should be brought to the appointment). If arrangements are made directly with the radiology facility to courier a patient’s films to our office for the appointment, it is the patient’s responsibility to make sure they have arrived for the appointment. Patients will not be seen without current (within 4 months of appointment) MRI scans.
What else do I need to know?
Dr. Fitzpatrick’s office is open Monday through Friday 8 a.m. until 4 p.m.. We do not close our office for the lunch hour. Patients are routinely seen Mondays, Wednesdays, and Fridays from l0:00 a.m. to 2:00 p.m. and Wednesdays from 9:00 a.m. to 3:30 p.m.. Surgery days are Tuesdays and Thursdays.
It is our office policy not to see patients who have had previous surgery unless the referring doctor speaks to Dr Fitzpatrick directly. It is then based on a case-by- case basis.
MRI's that are brought to an appointment can only be brought on film. We do not except images printed on CD's or paper
