Medical Records

If you would like to have a copy of your medical record sent to you or your physician, please print, complete, and sign the Authorization for Release of Medical Information and fax it to us at (602) 264-2417. We will mail the record to you as soon as we can. If the record is an older one, we may have to request it be sent from storage, which may take an extra two to three days.
