How do I make an appointment?

• The patient can call in directly or have the referring physician's office call to schedule the appointment.  The telephone number to schedule an appointment is (480) 425-8004. 
• The patient must have had an MRI within 4 months of the scheduled appointment and a referral if required by current insurance company.

What happens next?

• An appointment is scheduled, should appointment guidelines be met. 
• Patient will be asked to arrive 15 minutes prior to the appointment and hand carry the actual MRI films and report(s)  ~ we do not accept CD’s. 

What else do I need to know?

• Office hours are Monday – Friday, 8:00 a.m. – 4:00 p.m.
• Dr. Marciano’s clinic hours are Tuesday and Thursday,  8:00 a.m. – 4:00 p.m
